or bundle of His. Blockage of both the right and left bundle branches would be tantamount to block at these locations. This is the well-established concept of bilateral bundle-branch block.1-3 It highlights a hazard inherent in catheterization of the ventricle opposite that which is already the site of bundle-branch block. Since the advent of cardiac catheterization, numerous reports have been published of the induction of transient right bundlebranch block due presumably to fortutitouLs injury of the right bundle branch by the tip or side of an exploring catheter.4 16 If such a patient happened already to be affected with left bundle-branch block, right ventricular catheterization could then induce bilateral bundle-branch block and all the possible consequences of suddenly developed complete atrioventricular block. Wood7 reported the development of 2:1 atrioventricular block during right ventricular catheterization of a patient with pre-existent

Complete atrioventricular block induced during left heart catheterization.

Complete atrioventricular block (CAVB) during cardiac catheterization is a rare complication. We describe a patient with preexisting complete right bundle branch block who developed CAVB during left-sided cardiac catheterization. CAVB was induced when a left-sided catheter was passed through the aortic valve. We speculate that the patient's His bundle was injured by mechanical compression. Phys...

Electrocardiographic features and prevalence of bilateral bundle-branch delay.

BACKGROUND Definitive diagnosis of bilateral bundle-branch delay/block may be made when catheter-induced right bundle-branch block (RBBB) develops in patients with baseline left bundle-branch (LBB) block. We hypothesized that a RBBB pattern with absent S waves in leads I and aVL will identify bilateral bundle-branch delay/block. METHODS AND RESULTS Fifty patients developing transient RBBB pat...

Ventricular asystole during pulmonary artery catheter insertion.

669 September 2015 A the overall pulmonary artery catheter (PAC) use has decreased, it remains a valuable tool in select patients. PAC insertion may result in ventricular or supraventricular dysrhythmias due to contact of the catheter with the cardiac chamber walls. The use of balloon-tipped catheters during insertion serves to increase the catheter tip surface area, thereby mitigating applied ...

Iatrogenic Transient Complete Heart Block in a Preexisting LBBB

Catheter induced cardiac arrhythmia is a well-known complication encountered during pulmonary artery or cardiac catheterization. Injury to the cardiac conducting system often involves the right bundle branch which in a patient with preexisting left bundle branch block can lead to fatal arrhythmia including asystole. Such a complication during central venous cannulation is rare as it usually doe...

Bilateral Bundle-Branch Block

The electrocardiograms and salient clinical features of seven patients with electrocardiographic evidence of various types of bilateral bundle-branch block (BBBB) have been presented. When the complexes change in form and the P-R interval varies, the mechanisms are usually best interpreted by using the concepts of first degree, second degree, and third degree bundle-branch block with or without...
